The mission of the Heart Failure and Cardiac Transplant Department of Shaheed Rajaie Cardiovascular Medical and Research Center is to provide the best in innovative, evidence-based care for various cardiomyopathies, heart failure and pulmonary hypertension.

Multidisciplinary Heart Failure Care
The program encompasses a multidisciplinary approach to provide individualized and comprehensive care for heart failure and cardiomyopathy.
Science-based medication management for heart failure and cardiomyopathy.
Electronic device implantation and coordinated follow-up.
Education for patients and caregivers as part of comprehensive care.
Inpatient care for patients with advanced heart failure.
Short-term and durable mechanical circulatory support device implantation.
Cardiac transplantation and innovative treatment methods.
Pulmonary Hypertension Program
The pulmonary hypertension program provides evidence-based diagnostic and therapeutic care for patients with all types of pulmonary hypertension. Services include clinical and paraclinical assessments, diagnostic imaging such as echocardiography, nuclear imaging and multidetector CT scan, right heart catheterization, infusion unit care, and outpatient and inpatient services.
Team and Collaboration
The team includes three heart failure specialists, cardiovascular surgeons, nurse practitioners and coordinators, pharmacists, social workers, dieticians, psychologists and psychiatrists.
